Introducing the newest addition to the Michigan Opioid Collaborative

The Michigan Opioid Collaborative has recently expanded services to include education and case consultation on Hepatitis C Virus (HCV)! MOC welcomes Dr. Ponni Perumalswami, an associate professor of Internal Medicine in the Division of Gastroenterology and Hepatology at University of Michigan and the Ann Arbor VA Healthcare System, as our HCV expert. The expansion of the HCV services will support primary care and community providers with diagnosis, treatment planning, and medication management of people living with HCV.

Email MOC5 Project Coordinator, Audrey Hazelbaker, at ahazelba@med.umich.edu to set up an individual meeting with Dr. Perumalswami. At this meeting, we will determine how to tailor services to your patients and you.

Sign a Consulting Agreement

Before we begin the consultations, MOC will ask the provider to sign a brief consulting agreement. This agreement outlines the collaborative relationship and specifies that we do not collect, view, or discuss any personal health information during case consultations.

Submit HCV treatment cases for review

With each case, Dr. Perumalswami will send the provider a signed copy of the HCV consulting form to submit with the provider’s HCV treatment prior authorizations. Case consultations will be addressed via email within 48 business hours.

Join our biweekly HCV case conferencing

HCV case conferences will guide you through HCV treatment selection and optimal management. Cases submitted for review will be discussed in further detail at our biweekly HCV case conferencing. We meet Fridays 9-10AM EST every other week.
